Step by step
- 1
Decide what you want gone
Cancelling your Privacy Authorization ends ExtraCare rewards at the pharmacy but keeps your ExtraCare membership. A privacy deletion request goes further and deletes your data and linked accounts.
- 2
Open the pharmacy rewards opt-out page
Go to the ExtraCare Rewards at the Pharmacy opt-out page. It asks for your store number, Rx number and date of birth, all of which appear on a prescription label.
- 3
Call if you have no recent Rx number
If you don't have an Rx number from the past year, the page tells you to call 1-800-SHOP-CVS (1-800-746-7287).
- 4
Submit the cancellation
Submit the form. CVS says you can cancel the authorization at any time. Revoking it ends your rewards participation and removes accumulated credits from your ExtraCare record, per the ExtraCare terms.
- 5
To delete your data, send a privacy request
Use the privacy rights webform or phone line in the CVS privacy policy. CVS says a deletion also deletes your ExtraCare card, CarePass membership and CVS.com account.
Watch out for
- Cancelling the Privacy Authorization removes any rewards credits you've built up.
- Cancelling rewards doesn't delete your data. Only a privacy deletion request does that.
- CVS says it may keep some information for recordkeeping and to finish earlier transactions, so a deletion request may not clear everything.
- Older CVS documents describe a different authorization length and cancel link. Use the pages CVS shows you today.
What stays out there
Closing your CVS account stops new data collection at CVS. It doesn't reach copies that data brokers and people-search sites already hold, such as an old address or phone number tied to your name, or the profile a loyalty or marketing list sold on. Those sit outside CVS and need their own removal requests.
Serus finds where your details are listed and removes them, then watches for them coming back. If you want to check what's out there, run a free scan.

Yes. CVS's ExtraCare terms say revoking the authorization ends rewards participation and removes accumulated credits from your record.
How do I delete my CVS data entirely?
Send a privacy deletion request through the webform or phone line in CVS's privacy policy. Our guide to deleting a CVS account covers it.
